The Medicines Safety Programme has developed a series of indicators to inform safer prescribing practice, helping pharmacists, clinicians and patients to review prescribed medicines and prevent avoidable harm.
The purpose of the indicators is to identify prescribing that could potentially increase the risk of harm and that may be associated with hospital admission.
